Steps for a claim

  1. Initiating a Claim
    Fill out and submit the following form about the deceased to start the claims process.
  2. Verification within 5–7 business days
    If a benefit is payable following the death of the Owner or Annuitant, a claim packet will be mailed which include an applicable claim form, a letter showing the benefit amount, and options for claiming funds.
  3. Complete & Submit Claim Packet
    Once all claims requirements are received and are in good order, the benefit payment will be processed within 8-10 business days from when your paperwork was received at Âé¶¹´«Ã½'s home office. Claim forms can be submitted by email, fax or mail.
  4. Receive Payment
